These Terms of Service (these “Terms”) apply to your access to and use of the website at https://censuscrm.com (the “CensusCRM Website”) and the CensusCRM product (“CensusCRM”). CensusCRM is the customer relationship management (CRM) product offered by Census Health, Inc. (“Census Health,” “we,” “us,” or “our”). CensusCRM is a product name and brand of Census Health, Inc.; it is not a separate legal entity, and Census Health, Inc. is the provider of CensusCRM and the party to these Terms.
By accessing the CensusCRM Website or using CensusCRM, you agree to these Terms. If you are accepting on behalf of an organization, you confirm that you are authorized to bind that organization. If you do not agree, do not access the CensusCRM Website or use CensusCRM.
1. The Census Health Terms of Service Govern
Your use of CensusCRM and the CensusCRM Website is governed by the Census Health Terms of Service (the “Master Terms”), available at https://census.health/terms, which are incorporated into these Terms in full by this reference. These Terms supplement the Master Terms for the CensusCRM product. Except as expressly modified below, all provisions of the Master Terms — including those covering eligibility, acceptable use, intellectual property, privacy and HIPAA, disclaimers, limitation of liability, indemnification, governing law, and dispute resolution — apply to your use of CensusCRM and the CensusCRM Website.
Please read the Master Terms carefully. By using CensusCRM, you agree to be bound by them as if they were set out here in full. You can review the current Master Terms at any time at https://census.health/terms.
2. Product-Specific Terms
The following terms are specific to CensusCRM and the CensusCRM Website. Where any term in this Section conflicts with the Master Terms, this Section controls solely with respect to CensusCRM and the CensusCRM Website.
Product and Website. As used in the Master Terms, the “Service” means the CensusCRM CRM product and its related tools, applications, programming interfaces, and documentation, and the “Website” means censuscrm.com and its subdomains.
Scope of CensusCRM. CensusCRM provides the customer relationship management capabilities of the Census Health platform. Other Census Health products and capabilities are offered separately and may be governed by their own product-specific terms together with the Master Terms.
Subscription Agreement. Access to CensusCRM by a Customer is provided under a separate Subscription Agreement with Census Health, Inc. In the event of a conflict between these Terms or the Master Terms and a Subscription Agreement, the Subscription Agreement controls with respect to the product.
HIPAA and PHI. Where CensusCRM is used to create, receive, maintain, or transmit Protected Health Information, Census Health does so under a Business Associate Agreement executed with the Customer, as described in the Master Terms.
3. Contracting Party and Contact
CensusCRM is provided by Census Health, Inc. All rights, obligations, disclaimers, and limitations in the Master Terms run to and from Census Health, Inc. Questions about these Terms or the Master Terms can be directed to:

4. Changes to These Terms
We may update these Terms and the Master Terms from time to time. When we make a material change, we will update the “Last Updated” date above (and on the Master Terms) and, where appropriate, provide notice by email or through the CensusCRM Website. Your continued use of CensusCRM or the CensusCRM Website after a change takes effect constitutes acceptance of the updated Terms.
